Contract
- assert CSV.parse with headers: true returns a CSV::Table rather than an Array
- assert table[col_name] performs column-mode indexing returning an Array across all rows
- assert table.to_a prepends the header row at index 0 rather than returning only data rows
- assert CSV.generate with headers: [...] omits the header line unless write_headers: true is passed
- assert CSV.generate appending a Hash requires headers: option and matches key types strictly
- assert CSV::Row with header_converters: :symbol does not support indifferent string key access
