- pgx.NamedArgs rewrites named parameters into 1-based positional placeholders ($1, $2, ...) and produces ordered argument values
- pgx.NamedArgs reuses positional placeholders when the same parameter name appears multiple times in SQL without duplicating arguments

- pgx.NamedArgs supplies nil for parameters missing from the map without returning an error
