Contract
- StreamableHTTPClientTransport initializes with endpoint URL, custom options, and exposes sessionId getter
- Calling start() on an already started StreamableHTTPClientTransport throws an Error
- Outbound JSON-RPC requests send POST with application/json and mcp-protocol-version headers, and dispatch responses to onmessage
